- Date of birth
- What is their relationship to you?
- What industry do they work in?
- Occupation
- Driving licence number
- Length of car licence
- How many years claim free driving have they had on a motor policy?
- Any advanced driving qualifications?
- Does the person have access to another vehicle?
- Any motor claims in the last 5 years?
- Any driving convictions in the last 5 years?
